I've made a mistake as I evaluated PHYS 222 for this student and assigned 3 credit hours of PHYS 2XXX. Please replace that with 4 credit hours of PHYS 2211 assigned for BOTH PHYS 221 AND PHYS 222. Thanks.

I am trying to fill out the credit evaluation form for a physics class I had taken at my previous university. I have attached the syllabus below. The issue I am running into is that while this is a lab based physics class, there is not a separate course number for the lab nor a separate syllabus. I do not know how to fill out the lab portion of the evaluation form as it requires separate documents and other information which do not exist.
